AmeriHealth Mercy Health Plan to Present Care Coordination Strategies for Medicaid Members at Disease Management Congress

Harrisburg, PA - Margaret Angello, RN, Associate Vice President, Patient Care Management, AmeriHealth Mercy Health Plan will present on Utilizing Aggressive Care Coordination Strategies to Improve Outcomes for Medicaid Members with Multiple Co-Morbidities and Complex Social Issues, at the 10th Annual Disease Management Congress in Orlando, FL, on Thursday, September 22nd from 4:30 p.m.-5:00 p.m., at the Omni Orlando Champions Gate Resort.

Angello will discuss how AmeriHealth Mercy Health Plan launched an innovative Care Coordination program that includes Pediatric Preventative Care, Pregnancy Management and Intensive Care management to provide services to its most vulnerable members. By focusing on members who have multiple co-morbidities and complex social issues, AmeriHealth Mercy has seen measurable benefits through the implementation of comprehensive Care Coordination. Specifically, Angello will detail:

  • Processes used to develop an innovative Care Coordination Program
  • Strategies for identifying the members who will benefit the most from these services
  • Foundation for enhancing the traditional interdisciplinary team approach to complex medical management
