Care Coordination (formerly Keys to Your Care)

AmeriHealth Mercy Health Plan has special programs to help you stay healthy. You do not need a referral from your PCP to be a part of any of these programs.  If you have one of the health care conditions listed below, you could become a part of one of our special programs.

Care Coordination has programs for the following conditions:

There are a few ways you can be a part of one of these programs:

  • Your PCP, specialist or other health care provider may talk to you about becoming a part of the program.  He or she will call us to get you connected.
  • We may see from your health history that you might benefit from a program.  We will send you information in the mail about becoming a part of a program.
  • You can just ask!  Call Care Coordination at the number, below, and ask about any one of these programs.  

These programs help you better understand your condition. A care manager helps coordinate your health care and sends you information about your condition.

If you have extra needs, your care manager will work with you and your PCP.  You will set and work on personal goals to improve your health and quality of life.

As an AmeriHealth Mercy member, you have the right to say you do not want to be a part of one of these programs. You can tell us on the phone or in writing. If you do not want to be a part of one of these programs, it will not change your AmeriHealth Mercy benefits in any way.  It also will not change the way you are treated by AmeriHealth Mercy and our providers or the Department of Public Welfare.
