HIV/AIDS Case Management Program — Pathways

If you test positive for HIV or have been diagnosed with AIDS, AmeriHealth Mercy has a special program, called Pathways, to help you get the care you needIf you test positive for HIV or have been diagnosed with AIDS, AmeriHealth Mercy has a special program, called Pathways, to help you get the care you need.

AmeriHealth Mercy’s Pathways Program offers help to any member who tells us about their HIV status. Information about your medical condition is kept confidential. Please call Member Services to find out how to enroll in the Pathways Program.

Once you enroll in the program, your Pathways Case Manager will:

  • Assist you and your PCP/specialist with coordinating your health care needs
  • Help you identify and access providers, including local AIDS services organizations, to meet your special needs
  • Collaborate with the local AIDS service organization that you choose to ensure you get the medically necessary services you are entitled to receive
  • Act as a single point of contact for you at AmeriHealth Mercy

If a member chooses not to tell us about their HIV status, they can receive general Case Management services through AmeriHealth Mercy’s Case Management Department.  For more information on Case Management services, please call Member Services.
